Two patients' journeys through the rhinology clinic of the academic university hospital are reported. Over the course of six months, Case 1 suffered from nasal blockage. Computed tomography scans illustrated bilateral uncinate processes, curved medially and folded anteriorly, and a right middle turbinate concha bullosa, with its superior end exhibiting a medial inclination. A 29-year-old man, experiencing a nasal obstruction largely on his left side, presented for care after years of discomfort. The nasal endoscopy procedure demonstrated a split right middle turbinate and a significant lateral displacement of the nasal septum toward the left. Upon undergoing a CT scan of the sinuses, a duplication of the right middle turbinate was observed, taking the form of two middle nasal conchae.
Variations in anatomical structure, uncommon and rare, can arise at diverse points during embryological development. Double middle turbinates, along with accessory, secondary, and bifurcated inferior turbinates, represent uncommon anatomical variations. Clinically, a double middle turbinate is a condition that is seen in only 2% of patients presenting to rhinology clinics. Despite a comprehensive search of the medical literature, only a few case reports were found about the phenomenon of the double middle turbinate.
Significant clinical consequences are associated with having a double middle turbinate. The structural differences in the body's anatomy may lead to a decrease in the size of the middle meatus, which can predispose the patient to sinusitis or perhaps contribute to secondary symptoms. Rarely encountered cases of middle turbinate duplication are part of our findings. The importance of appreciating the differences in nasal turbinates cannot be overstated for the diagnosis and management of inflammatory sinus conditions. A deeper exploration of the connection between additional medical conditions and the current situation requires further research.
Important clinical considerations arise from a double middle turbinate. Differences in middle meatus anatomy could lead to a narrowing, making the patient more susceptible to sinusitis or the possibility of associated secondary symptoms. Hepatic epithelioid hemangioendothelioma (HEHE) is a rare disease, consequently sometimes having a misdiagnosis.
